Issued after 1920.  Established in 1915 in Warsaw, Petkow, Wolbrom.  Two piece construction, white and red enameled cross with silver crowned eagle superimposed in the center.  Mini Order of the Virtuti Militari on the chest of the eagle.  Unit was located at Augustow. Original maker was J. Mirowski in Warsaw.
